Getting to medical appointments

MATP means Medical Assistance Transportation Program. It can help eligible Medical Assistance (Medicaid) members travel to covered care. These original documents explain the program or provide a blank application.

Keep personal information private. The application is a blank form to complete and return through the local MATP office's instructions. This website does not accept completed applications. Do not email Social Security numbers, medical information or completed forms to the Family Navigator.
